You are hereby authorized to use my survey response/Google review, recordings, photographs/video and/or reproduction of photographs/video of me/us, of my child/children, and of my/our property. These can be used in the following formats or other media, not specifically mentioned, for the purpose of advertising and publicity of your services:
- Electronic Media (website, Facebook, YouTube, etc.)
- Podcast Episodes
- Brochures, flyers, and other collateral materials
- Newspaper and/or magazine advertising
- Direct mail pieces
- Annual Report
IF AN ARTICLE IS WRITTEN:
You will be provided a copy of the article about your operation/business for review, which will be published in a Horizon Farm Credit publication or on the website.
If you have any edits or extensive changes, please coordinate them in a timely fashion directly with the writer/editor. You will receive an updated proof for review and approval separate to this form.
The form acknowledges that you are aware of the approval process and that pending your confirmation of final approval, Horizon Farm Credit is authorized to use the above noted items.
Important Notice Regarding Third-Party Solicitations:
Horizon Farm Credit will never charge you for copies of articles, photographs, framed features, or related materials created as part of this authorization. Any emails, letters, phone calls, or other communications offering to sell framed items, copies of articles or photos, or similar materials are not affiliated with Horizon Farm Credit and should be disregarded. If you are unsure whether a communication is legitimate, please contact comms@horizonfc.com.
Disclosure: This form must be completed in its entirety before the Horizon Farm Credit staff can proceed with distribution of the media collected. By filling out the form below, you authorize Horizon Farm Credit use of collected media (outlined above) in promotional and advertising messaging.
